OVERVIEW

After a focus group session, we realised that we needed to create a consistent referral process to get new patients onboard. The general process was previously unsecured in terms of data handling, and it was different wherever the referral was coming from. We gathered experiences from nurses and individuals working in the industry to build this while answering both the business and the referrers’ needs.

WHY ARE WE BUILDING THIS?

• The data collected is not always coherent or consistent in comparison to what is required

• Reach business requirements by getting more referral in a shorter time through one single portal

• There was no clear visibility on when and where the referral was created, this could potentially induce waiting time for patients wanting to do the programme

CHALLENGES

• Clinical specialist nurses usually lack time to refer patients

• There could be a lack of trust from the clinicians providing us patients informations, as they are held accountable

• This process would need to be adopted by all the clinical staff and other referrers in order to refer patients, therefore it should be accessible for them

GOALS

• Create a secure platform capable of handling patients’ data with industry standards

• Respect the responsiveness of the platform, allowing the form to be used on both mobile and desktop

• Collect the right data to get enough information on the patients’ diagnosis and their contact details
